New Employee Onboarding List for IT

IT department must have all the required information to properly set up a new employee.
Here's a list of information that hiring managers should submit to the IT department for onboarding a new user:

General Information
  1. Full Name
  2. Department
  3. Job Title
  4. Start Date
  5. Office Location
  6. Manager's Name
  7. Contact Information (email, phone)
Hardware Requirements
  1. Laptop/Desktop specifications
  2. Additional monitors
  3. Any special hardware (e.g., high-performance workstations, graphic cards)
  4. Phone extension
  5. Mobile device
Software Requirements
  1. Specialized software (e.g., design software, analytics tools)
  2. VPN Access
  3. Traverse access required.
  4. VAT and MyVantage access required.
  5. Email distribution lists to join.
  6. Communication Tools.
  7. ZAC and Zultys call groups.
  8. Internal communication platform (Teams, etc.)
  9. Video conferencing tools
Network and Security (Default - same as the dept)
  1. Level of network access required. 
  2. File/Folder permissions.
  3. Impress access required. (What menus)
  4. Two-Factor Authentication setup -  All remote employees must have the Microsoft Authenticator App installed to access Vantage remotely.
Training and Documentation
Cybersecurity awareness auto enrolled within first week. Notify user to take Sophos training. 
Relevant documentation to be provided.
Additional Notes
Any specific configurations or custom setups
Temporary or contract employee details, if applicable
